Hello FHS LAX Families & Players,
This is a reminder that the Spring LAX season is almost upon us!
Please make sure your player has an updated physical form on file with the FV Athletics Office so they can obtain their green slip to play spring sports.
Official Sign-up for FV Spring Sports email will go out around 2/16/16.

Your player will need to pick up their green slip from the athletics office prior to our 2/29/15 “official spring sport season start”.
Coaches want all players on the field 2/29/15 with no excuses so please get those physicals done now! If they do not have a green slip, they are not allowed to be on the field or at practice.
You can scan & email the completed physical form to Adriana Torres our Athletic Director Asst. if it’s easier than taking to school in person. Her email address is adriana.torres@bvsd.org. Just let her know your player’s name, that they are playing Spring Lacrosse. Your player may need to remind her when they pick up their green slip that the physical form was emailed to her.
If you can’t get into the doctor - Walgreens & King Soopers in Lafayette both do physicals at their Healthcare Clinics. You can drop in or make an appointment online. Cost typically runs from $39-$60.
